I've followed the instructions in the Styleguide to a T but the EPUB version from Meatgrinder just won't create a working TOC. Funnily, the Calibre EPUB version works fine, so does the MOBI. I think I'm going to have to just get rid of my TOC if I want it published. I work in a publishing office as well as being an indie author, and am keenly aware of copyright issues. It's frustrating enough when I'm at my day job and have to explain to someone who is being stubborn about not being able to just grab an image from Google and print it on our publications -- so seeing authors with minimal graphic design experience grabbing images randomly off the Internet and using them as covers only exacerbates the feeling of frustration, because in the end, as you say, it's all of us who get branded if the industry finds someone to make an example of.
